By completing this form, I give my consent to

(hereinafter referred to as the Operator) to process personal data under the following terms and conditions:
1. Purpose of personal data processing:
recruitment for vacant positions.
2. List of personal data subject to consent for processing:
full name; contact phone number; email address; gender; company name; job title; information on work experience; information on education.
3. The Operator is authorized to perform the following actions with the personal data listed above through automated processing and processing without the use of automation tools (underline as applicable): collection, recording, systematization, accumulation, storage, updating (correction, modification), retrieval, use, transfer (provision, access), blocking, deletion, destruction.
4. I give my consent to the Operator to entrust the processing of my personal data to
for the purpose specified in Clause 1 of this Consent.
5. Consent is granted for a period of 5 (five) years from the date the purpose of personal data processing is achieved. Another period may be provided for by the legislation.
6. Consent may be withdrawn by me at any time based on my written application. The written application must be sent to the Operator by registered mail with return receipt requested or delivered in person against a signed acknowledgment to a representative of the Operator. In the event of withdrawal of this Consent, the Operator is entitled to continue processing personal data if grounds exist.
